For patients with pre-diabetes or diabetes, we have several services to help you live well while managing your disease. From diagnosis to treatment to implementing lifestyle changes, we are committed to helping you live at your fullest health potential.
You could have prediabetes or type 2 diabetes and not know it. If you have any of these risk factors, ask your doctor about getting your blood sugar tested:
- Overweight
- 45+ Years Old
- Family History
- Physically active less than three times/week
- History of gestational diabetes
- Non-alcoholic fatty liver disease
Moving Forward with Diabetes Program
Diabetes? Now what?
If you have been diagnosed with Type 1 diabetes, Type 2 diabetes, gestational diabetes, or glucose intolerance, this program is for you!
You don’t have to do it alone! Let us help you live at your highest possible quality of life and avoid more costly healthcare with our Moving Forward with Diabetes program. You will receive:
- Education about your condition
- One-on-one assessments with our Certified Diabetes Educator (CDE)
- Group educational classes and support
- Coordination with your medical provider
Program Timeline
Month 1
During your first month, you’ll have an individual appointment with our CDE to do an initial assessment and answer any questions you may have.
Month 2
In the second month, you’ll attend 4 weekly group educational classes where you’ll learn about diet, exercise, medication, management, possible complications, and more in a supportive setting where you can connect with others like you.
Month 3
In your third and final month, you’ll have another individual appointment with our CDE for assessment and to advise you on how to incorporate all you’ve learned into daily life.
Enrollment Progress
1. Provider Referral
Your provider can refer you to Moving Forward Diabetes Program by faxing the referral form to (828) 266-1065. Our staff will contact you to schedule your first appointment
2. Insurance Pre-certification
Our staff will determine if you are eligible for insurance coverage, inform you of the coverage amount, and enroll you in the program.
3. Attend the Program
Be sure to attend your initial appointment with our CDE, group sessions, and the final CDE appointment. Once you’ve finished Moving Forward, we can recommend other programs to help you maintain your level of health.
Medical Nutrition Therapy
Individual nutrition counseling, with a qualified registered dietitian/nutritionist (RDN), is available for patients with a variety of conditions including diabetes, renal disease, heart disease, gastrointestinal disorders, malnutrition and obesity. Referral is required. Located at Watauga Medical Center and Cannon Memorial Hospital.
Referrals can be made via EPIC or by faxing the Referral for Medical Nutrition Therapy [PDF] to Clinical Nutrition at (828) 268-9397.
